What it does
Keeps a strip of the image in focus and blurs the rest away, the trick that makes real scenes look like miniatures.
The trick works because depth of field is not neutral information. Photograph a model train up close and physics forces most of the frame out of focus; photograph a real valley and everything is sharp from front to back. The brain has internalized that correlation so thoroughly that adding macro-style focus falloff to an aerial photo overrides every other cue in the picture, including the ones that should give the game away, like people and traffic. A high vantage point helps, because miniatures are usually seen from above. Boosted saturation helps too, because toys are painted brighter than the world.
On a real camera the effect requires a lens that pivots. Tilting the lens relative to the sensor tips the plane of focus, so instead of a wall of sharpness parallel to the camera it becomes a slice cutting diagonally through the scene. Architectural photographers use the movement to get more in focus, laying the slice along a building face. The miniature look abuses the same movement in reverse, angling the slice so almost nothing stays sharp except one shallow band. Done in software, the band is simply drawn: positioned, widened, feathered, and tipped so one side falls off faster than the other, which is the two-dimensional signature of a tilted focus plane.
What separates a convincing fake from a filter is how the blur grows. Real defocus increases progressively with distance from the focus plane, so the blur has to ramp smoothly, not switch on at a masked edge. It also has to behave like a lens: bright points should bloom into soft aperture-shaped discs instead of smearing into gray, because bokeh is half of what shallow focus looks like. A circular focus region does for a single subject what the band does for a landscape, shrinking one building or one figure into a diorama piece. And video is where the effect earns most, since sped-up motion is the final ingredient of the miniature illusion.

What is the tilt-shift effect?
It is a selective-focus treatment that keeps a narrow region of the image sharp and blurs the rest with a smooth ramp, imitating the extremely shallow depth of field of close-up photography. Applied to a full-size scene, usually one shot from above, it makes the scene read as a miniature model. It can be produced optically with a tilting lens or synthetically with a graduated blur.
Why does tilt-shift make things look miniature?
Because depth of field scales with subject distance. Macro subjects inches from the lens get razor-thin focus, while distant landscapes come out sharp everywhere. The brain uses that correlation as a scale cue, so imposing macro-style focus falloff on a cityscape convinces the eye it is looking at something small, even when people and traffic say otherwise.
Where does the name tilt-shift come from?
From view-camera lens movements. Shift slides the lens parallel to the sensor to fix converging verticals in architecture, and tilt pivots it to reposition the plane of focus. Only the tilt produces the miniature look, but lenses that offer one movement usually offer both, so the pair became the name of the effect.
What makes a fake tilt-shift look real?
Three things. The blur must increase progressively with distance from the focus band rather than switching on at a hard mask edge. Out-of-focus highlights should bloom into bright aperture-shaped discs the way a lens renders them, not average into gray mush. And the source has to cooperate: a high angle looking down, and ideally sped-up motion, since miniatures are viewed from above and small things appear to move quickly.

Yes, and video is where the illusion completes, because motion carries the last scale cue: small things seem to move fast, which is why the most famous tilt-shift pieces are timelapses.
